This document outlines the terms, conditions, and informed consent for engaging in counselling and/or coaching services with The Revived Life Counselling & Coaching ("The Revived Life"). By engaging in services and acknowledging this agreement via checkbox, you confirm that you have read, understood, and agreed to the following.

1. Nature of Services: Counselling and Coaching

The Revived Life provides both counselling and coaching services. While these approaches may share tools and reflective processes, they are distinct:

  • Counselling focuses on emotional support, personal insight, and psychological wellbeing. It may involve exploring past experiences, patterns, and emotional responses.

  • Coaching is future-focused and goal-oriented, supporting clients to clarify goals, develop strategies, and take action. Coaching does not diagnose or treat mental health conditions.

Where appropriate, elements of both may be used; however, this service is not a substitute for psychological, psychiatric, medical, or crisis support.

2. Scope and Limitations of Service

The Revived Life works with adults aged 18 years and over.

The Revived Life does not provide services for:

  • Severe or complex mental illness

  • Psychosis

  • Crisis intervention

  • Drug and alcohol dependency

  • Couples counselling

If at any point your needs fall outside the scope of practice, you may be referred to another appropriately qualified professional.

3. Client Responsibility and Informed Consent

You acknowledge that:

  • You are responsible for your own decisions, choices, actions, and outcomes.

  • Counselling and coaching require active participation and honesty.

  • Outcomes cannot be guaranteed.

You agree to disclose relevant information about your physical, mental, and emotional health that may impact your work together.

You may discontinue services at any time.

4. Sessions, Fees, and Cancellations

  • Sessions are 60 minutes in duration.

  • The fee is $100 per session.

  • Coaching packages may be discussed and agreed upon separately.

Cancellations:

  • A minimum of 24 hours’ notice is required for cancellations or rescheduling.

  • Sessions cancelled with less than 24 hours’ notice will be charged at the full session fee.

5. Mode of Delivery and Telehealth Consent

Sessions may be delivered:

  • In person

  • Via telehealth (video or phone)

By consenting to telehealth, you acknowledge:

  • Telehealth has limitations, including technology failure and reduced ability to assess non-verbal cues.

  • You are responsible for ensuring privacy at your location.

  • Telehealth is not suitable for crisis situations.

6. Confidentiality and Privacy

All information shared is treated as confidential and managed in accordance with:

  • The Australian Counselling Association (ACA) Code of Ethics

  • Applicable Australian privacy legislation

Confidentiality may be breached where required by law, including:

  • Risk of serious harm to yourself or others

  • Mandatory reporting obligations

  • Court orders or subpoenas

7. Record Keeping and Access

  • Client records are stored using practice management software and paper files.

  • Records are retained for 7 years in line with professional requirements.

  • Access to records is at the practitioner’s discretion, subject to legal and ethical obligations.

8. Crisis and Emergency Situations

The Revived Life is not a crisis service.

If you are at immediate risk or experiencing a mental health emergency, you are advised to contact:

  • Or attend your nearest emergency department

9. Professional Standards

Services are provided in accordance with:

  • Australian Counselling Association (ACA) ethical standards (Level 2 Member)

  • International Coach Federation (ICF) core competencies (coaching qualification completed; accreditation in progress)

10. Complaints and Concerns

If you have concerns about your service, you are encouraged to discuss them directly with The Revived Life in the first instance.

If concerns cannot be resolved, you may lodge a complaint with:

  • Australian Counselling Association (ACA)

  • International Coach Federation (ICF) (for coaching-related matters)

11. Limitation of Liability

To the extent permitted by law:

  • No guarantees are made regarding outcomes.

  • Liability is limited to the amount paid for services.

  • The Revived Life is not responsible for indirect or consequential loss.

12. Agreement and Acknowledgement

By acknowledging this agreement via checkbox, you confirm that:

  • You have read and understood this document

  • You consent to counselling and/or coaching services under these terms

  • You understand your rights and responsibilities
